Above all else, their Continuity of Care Pathway is the type of support that makes transitioning to every stage of care stress-free, including moving a patient to the nursing home.

Plus, they have trusted contacts, like nursing home partners, that make the transition a breeze. Although we did receive a slightly delayed response from the management, which wasn’t a regular occurrence, thank goodness.

The level of care doesn’t diminish, by the way. From what we’ve seen, they’ve provided quality care filled with compassion and support.

Their services include medical and non-medical home care in Ireland, focusing on live-in support. They offer three carer options: Carer Plus with 1 to 3 years of experience, Advanced Carer with over 3 years, and a qualified nurse (live-in nursing).

Furthermore, their carers are well-equipped to assist with meal preparation, medication reminders, exercise, domestic tasks, and socialisation. They can visit up to 7 days a week, and although live-in care isn’t available, they do come around 1 to 3 times a day.

A service that’s worth highlighting is their non-medical home care for mothers, which isn’t something you normally hear. Basically, a carer will drop by to help with household tasks and errands, because birthing a child is no easy feat.

What we’re grateful for The Care Team for living up to their name, as every single step is handled and delivered very thoughtfully. The customised home care plans are handled by their client supervisors, who coordinate with registered nurses.

They also offer loads of options for every situation, including visiting and live-in care. There’s home help for daily activity assistance, respite care as a temporary break for the patient’s family, and overnight care for those needing help with mobility at night.

We want to highlight their live-in care that can provide relief to family carers. The best bit is the customised plan that targets the patient’s physical and mental well-being, as well as their independence.

We saw no issues with their home health care providers, but gaps in communication with the management soured some clients’ experiences.

It’s their healthcare specialists who really shone in providing top-notch physical and emotional support. It was hardly surprising, as they received hands-on training and understood the importance of showing compassion to patients with mobility challenges.
